ADAMTS13 First: A Board-Ready Approach to TTP, HELLP, and Pregnancy-Associated TMA

When severe thrombocytopenia and hemolysis appear in pregnancy, use a syndrome-first algorithm, ADAMTS13 intelligently, and targeted retrieval practice to separate TTP from HELLP and complement-mediated TMA.

In an illustrative board-style stem, a pregnant patient with profound thrombocytopenia, microangiopathic hemolytic anemia, and organ injury is not automatically a HELLP question. The board-level move is to name the syndrome first—pregnancy-associated thrombotic microangiopathy (TMA)—then identify the discriminator.

In a stem with severe thrombocytopenia, marked hemolysis, schistocytes, and an ADAMTS13 activity of 6%, the high-value clue is severe ADAMTS13 deficiency, a high-specificity finding that strongly supports TTP in the compatible clinical context. Mild transaminitis can occur in TTP; it should not outweigh this high-specificity result.

Start with the syndrome, not the pregnancy label

On the first pass, confirm three elements:

  • Thrombocytopenia: How severe is it, and is there a prior platelet count for comparison?
  • Microangiopathic hemolysis: Look for schistocytes, elevated LDH, low haptoglobin, indirect hyperbilirubinemia, reticulocytosis, and a negative direct antiglobulin test when provided.
  • Organ injury: Is the dominant problem neurologic, renal, hepatic, cardiac, pulmonary, or obstetric?

Then ask what the stem has not told you. The supplied pattern does not provide blood pressure, proteinuria, baseline creatinine, baseline platelets, headache or visual symptoms, right-upper-quadrant pain, urine output, prior TTP, family history, or the postpartum course. Do not silently invent those facts.

Those omissions matter. Preeclampsia with severe features requires a hypertensive context, although proteinuria is not required when qualifying end-organ findings are present. ACOG severe-feature criteria include platelet count below 100 × 10⁹/L, creatinine above 1.1 mg/dL or doubling from baseline, liver transaminases more than twice the upper limit of normal or persistent severe right-upper-quadrant or epigastric pain, pulmonary edema, new-onset headache or visual symptoms meeting the relevant criteria, and severe-range blood pressure, defined as systolic blood pressure of at least 160 mm Hg or diastolic blood pressure of at least 110 mm Hg.

Those are severity criteria for hypertensive disease—not a synonym for HELLP and not a diagnostic requirement for TTP. For example, an AST only slightly above the laboratory reference range is not the ACOG liver threshold for a severe feature and, by itself, does not establish complete HELLP. It also does not exclude a partial or evolving HELLP phenotype.

Use ADAMTS13 as a discriminator, not decoration

A complete HELLP phenotype is classically defined by hemolysis, elevated liver enzymes, and low platelets. Partial or evolving HELLP patterns are described, but they should not be treated as interchangeable with complete HELLP. TTP and complement-mediated TMA can produce the same broad laboratory pattern, so the question is usually asking which clue has the greatest specificity.

An ADAMTS13 activity below 10% of normal strongly supports TTP when thrombocytopenia and microangiopathic hemolysis are present. It does not by itself distinguish immune-mediated from congenital TTP, and assay timing matters; ideally, the sample is obtained before plasma therapy.

Complement-mediated TMA, often termed atypical hemolytic uremic syndrome (aHUS) in clinical literature, rises on the differential when kidney injury is severe, renal-predominant, persistent, or worsening after delivery and ADAMTS13 is not severely deficient. It remains a diagnosis of exclusion. Renal involvement alone does not establish that diagnosis, because TTP and hypertensive disease can also affect the kidneys.

The management sentence that matters

If clinical suspicion for TTP is high, treatment should not wait several days for a send-out ADAMTS13 result. The practical board answer is to obtain the diagnostic sample before plasma-based therapy when feasible, involve hematology and transfusion medicine, and initiate urgent therapeutic plasma exchange with specialist-directed corticosteroid therapy. Delivery may be necessary for obstetric indications, but delivery alone is not the treatment for TTP.

Conversely, if a presumed HELLP presentation does not begin to improve after delivery or worsens postpartum—especially if severe thrombocytopenia, hemolysis, or kidney injury persists beyond approximately 48–72 hours—do not keep repeating the same label. Reopen the differential for TTP, complement-mediated TMA, disseminated intravascular coagulation, acute fatty liver of pregnancy, and other causes of hemolysis and organ injury.

MCQs that ask which statement is accurate often combine diagnosis, expected course, and treatment in one option. Read each option clause by clause. A statement that correctly identifies TTP but says delivery alone is definitive is still wrong.

Turn one question into retrieval practice

Rereading the explanation creates familiarity. Retrieval practice requires producing the contrast before looking at the answer. Use this seven-minute revision exercise after completing a question like this one:

  1. Minute 1: From memory, write the TMA triad and list the missing history or examination data that would affect the formulation.
  2. Minutes 2–3: Recreate a three-row comparison for TTP, HELLP, and complement-mediated TMA using only three headings: discriminator, dominant organ pattern, and immediate management signal.
  3. Minutes 4–5: Perturb the case. Change ADAMTS13 to a nonseverely reduced value, change the clinical picture to persistent severe postpartum kidney failure without another explanation, or add severe-range hypertension with transaminases above twice the upper limit. For each version, state the leading diagnosis and the next action.
  4. Minute 6: Write one sentence beginning with “The best answer is ___ because ___.” This exposes whether your reasoning depends on a threshold, a mechanism, or an assumption.
  5. Minute 7: Record one error in wording, not just the correct diagnosis: anchoring, threshold neglect, missing-history invention, or management mismatch.

Repeat the drill from a blank page 48–72 hours later. The goal is not to memorize a table; it is to recognize which variable changed and whether that change should alter the diagnosis, the urgency, or both.
